We consider it a privilege that you have chosen us for your cosmetic needs. We feel strongly about informing you regarding all aspects of your care and in doing so, would like you to be aware of our financial policy.
We charge a $75.00 consultation fee and we require a credit card at time of scheduling. We understand that situations arise in which you must cancel your appointment. Patients who do not show up for their appointment without a call to cancel an office appointment at least 24 hours prior will be considered as a no-show. All no-shows will be charged a $250 fee. This fee can be credited towards a selected procedure in the future but arrangements must be made with the practice. Surgery must be scheduled one year from the date of your consultation.
We understand that Special unavoidable circumstances may cause you to cancel within 24 hours. Fees in this instance may be waived but only with management approval.
Our practice firmly believes that good physician/patient relationship is based upon understanding and good communication. Questions about cancellation and no show fees should be directed to the Billing Department at (425) 455-0444.
Scheduling a case requires a nonrefundable deposit of $1,000. This fee will be credited towards your surgery if scheduled within 6 months of the consultation.
Payment for surgery is due at your pre-operative appointment or 2 weeks prior to your surgery/procedure – whichever comes first and is non-refundable. If your balance due is not paid at that time, a hold will be placed on your surgery/procedure and your surgery date will become open for scheduling.
The pre-operative appointment is a final consultation prior to your surgery/procedure. The fee paid at the time of your pre-operative appointment is non-refundable and non-transferable. If surgery is rescheduled, a 35% rescheduling fee will be charged. This is done to maintain the continuity of a very valuable and busy surgical schedule. Exceptions to this policy include life-threatening circumstances, but the office reserves the right to exercise discretion in implementing this policy.
It is the patient’s responsibility to complete all of the pre-surgical diagnostics prior to the preoperative appointment. If not completed by this appointment, surgery may be postponed and subject to a 35% rescheduling fee, or cancelled without a refund.
Every effort is made to maintain the predicted operating room and anesthesia hours. However results are NEVER compromised for time. In these rare cases where the surgery outruns the determined time, you will be billed for additional operating room and anesthesia fees.
Plastic surgery is an art and occasionally revisions are necessary. Revision surgeries must be scheduled with 1 year of the initial surgery. Although the surgeon’s fee may be waved, facility and anesthesia fees will apply and are the responsibility of the patient. Dr. Stephens reserves the right to determine what is considered a revision surgery and a stand-alone procedure.
All returned checks have a $35 service charge.

Due to the time involved in describing the elective nature of cosmetic procedures and because we fully inform you of the limitations and risks associated, we do not provide refunds for any reason.
