Full Facial Rejuvenation:
Dr. David Stephens has been performing the “deep plane” facelift procedure since the late 1990s for patients who require surgical correction of the face and neck. Dissecting under the “deep plane,” he will modify the procedure in order to address the individual’s specific, unique anatomic deficiencies while shaping and contouring the face into a more beautiful, rested, and younger appearance with natural facial contours. You will look like a refreshed version of yourself, a natural, unoperated look.
The “deep plane facelift” is not a new procedure. Advancements in facelift techniques over the past 20-30 years have enabled competent surgeons to provide consistently beautiful outcomes using established and safe techniques. The Bespoke Facelift addresses the specific areas where correction is necessary while supporting the facelift with a stronger deep tissue lift rather than the skin. You may need more deep sculpting of the neck, more or less cheekbone shaping, treatment of the lower eyelid bags, upper eyelid surgery, a brow lift, upper lip shortening, and improvement of the jowls and lower face and cheeks.
The Short Scar Facial Rejuvenation Procedure:
Candidates for the short scar facelift are typically younger individuals or Asian and African skin types, where there is less skin laxity in the face and jowls and minimal skin laxity in the neck.
A deep plane contouring of the face is still done, but the incisions are tailored to be shorter. Dr. Stephens will contour the facial structures, addressing the loss of fullness of the cheekbones through the lift or fat grafting the cheekbones. He may also reduce the jowls by removing or contouring the deeper buccal fat. Also known as a limited-incision facelift or mini facelift, a short scar facelift is a powerful surgical procedure that addresses signs of facial sagging using smaller incisions to rejuvenate the face in a subtle, natural-looking way.
Endoscopic Facial Rejuvenation
Dr. Stephens has performed endoscopic brow lifts and midface rejuvenation since his time as a plastic surgery fellow in the 1990s, in a team led by the originator of the technique, Dr. Luis Vasconez. The technique uses hidden incisions and the magnified view of the endoscope to dissect and lift the deep structures in the upper face and midface. The best candidates for this procedure have excellent skin tone and elasticity with minimal skin laxity. This is an excellent procedure in younger Asian and African skin types, who are at more risk of scarring. It can also be combined with a neck lift using an incision under the chin.

How Are These Procedures Performed?
A facelift is an outpatient procedure performed in Dr. Stephens’ office-based licensed surgical facility (Ambulatory Surgery Center), under light general anesthesia with his team and our board-certified anesthesiologist.
Dr. Stephens begins by making small incisions hidden in the hairline. In the short scar facelift, there is no scar behind the ear and the incision starts at the sideburn, while in the classic deep plane face and neck lift, the incision is extended behind the ear and into the hairline. He then carefully undermines the skin and performs a dissection, releasing the strong SMAS tissue. He manipulates the underlying deep tissue and elevates the fat pads while contouring the deeper structures and securing them in place for lasting support. The skin is then artfully redraped, and any excess skin is trimmed away. Finally, incisions are delicately closed with strategic layers of tiny, precise sutures to promote optimal healing.
What Procedures Can I Combine With My Bespoke or Short Scar Facelift
Patients often choose to combine other procedures for more comprehensive rejuvenation. These might include:
- Upper or lower eyelid surgery to address eyelid bags or skin excess for brighter, more refreshed eyes

What Is the Recovery Like?
With newer techniques and medications that reduce bruising, patients typically experience minimal bruising and discomfort, which usually subside within about 2-3 weeks.
Rest and head elevation in a recliner are important during the initial recovery period. Gentle movement, such as short walks around the house, is recommended to promote circulation. Heavy lifting and strenuous activity are prohibited for about 3-4 weeks.
